Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

As a Physical Therapist (PT) in Home Health, you’ll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections. Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you’ll play a key role in helping clients achieve their highest level of independence and well-being.
As a Physical Therapist, you will:
Provide Direct Client Care – Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders. Deliver high-quality care that empowers clients to achieve their functional goals. Perform re-evaluations as required to assess progress and adjust care plans accordingly.
Maintain Accurate Client Records and Billing – Document all services accurately and promptly using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines.
Develop and Support Follow-Up Programs – Recommend and oversee continuation programs to support long-term client’s success and optimize discharge outcomes. Make referrals to continuation programs as necessary to promote maximum independence and functionality.
Communicate Effectively and Collaboratively – Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.

This job also requires:
Current Physical Therapist license in the state of Minnesota.
Strong collaboration and teamwork skills.
Clear and compassionate communication abilities.
Proficient in English for effective communication with staff, clients, and visitors.
Successful completion of a state-mandated background check.
Able to physically perform the duties required by the position.
Must obtain CPR certification within 6 months of hire.
