- Qualifications
✔ Active Physical Therapist (PT) license in the State of New Mexico or eligibility for licensure
✔ Master's Degree in Physical Therapy or equivalent qualifying degree
✔ Strong assessment, treatment planning, and patient education skills
✔ Ability to work independently in home and community settings
✔ Valid Driver's License and reliable transportation
✔ Excellent communication and documentation skills
- Responsibilities
Evaluate, assess, and develop individualized treatment plans for patients
Provide therapeutic exercises, functional mobility training, gait training, balance training, and manual therapy
Perform home safety and mobility assessments
Educate patients and caregivers on treatment plans and chronic disease management
Monitor patient progress and modify treatment plans as needed
Complete accurate and timely clinical documentation
Collaborate with physicians, nurses, caregivers, and interdisciplinary healthcare teams
Recommend adaptive equipment, orthotics, and mobility devices when appropriate
Promote patient safety and fall prevention strategies

We are seeking a compassionate and dedicated Physical Therapist (PT) to provide in-home rehabilitation services to adult and geriatric patients throughout Silver City, NM.
