We are looking for a dedicated Contract Physical Therapist to provide in‑home therapy services to adult and geriatric patients. The ideal candidate is reliable, clinically strong, and comfortable working independently in the field.
\nResponsibilities
\nPerform comprehensive physical therapy evaluations
\nDevelop individualized treatment plans based on patient needs
\nProvide therapeutic interventions to improve mobility, strength, balance, and functional independence
\nEducate patients and caregivers on exercises, safety, and home programs
\nDocument visits accurately and timely in EMR
\nCollaborate with interdisciplinary team members (RN, OT, ST, MD)
\nCommunicate changes in patient condition promptly
\nMaintain compliance with state and federal regulations
\nQualifications
\nActive Physical Therapist (PT) license in the state of practice
\nCurrent CPR certification
\nHome health experience preferred but not required
\nStrong clinical assessment and communication skills
\nReliable transportation and ability to travel between patient homes
\nAbility to work independently and manage a flexible schedule
