    Physical Therapist Rehab Clinical Coordinator

    Location: Margate, Florida – Office-Based

    Position Type: Full-Time (Monday–Friday, 9:00 AM – 5:00 PM)

    Position Summary:

    The Physical Therapist Rehab Clinical Coordinator is responsible for providing clinical leadership and support to Physical, Occupational and Speech Therapists delivering services in the home care setting. This office-based role focuses on ensuring high-quality patient care through clinical guidance, documentation oversight, and collaboration in plan of care development. The Rehabilitation Supervisor serves as a key resource for therapists, promoting compliance, clinical excellence, and best practice standards.

    Key Responsibilities:

    Provide clinical supervision and guidance to home care Physical, Occupational and Speech Therapists

    Review therapist documentation for accuracy, completeness, and regulatory compliance

    Assist clinicians with development, modification, and optimization of patient Plans of Care

    Answer clinical questions and provide ongoing mentorship to field therapists

    Ensure adherence to agency policies, state regulations, and payer requirements

    Collaborate with interdisciplinary team members to support patient outcomes

    Monitor clinical performance and identify opportunities for improvement and education

    Support onboarding and training of new rehabilitation staff as needed

    Participate in case conferences and clinical meetings
