Physical Therapist

Job Description

We are seeking a Physical Therapist to support a skilled nursing and post-acute care center located in the heart of San Francisco. This urban facility provides rehabilitation services for orthopedic recovery, medically complex cases, and mobility decline.

As the PT, you’ll conduct evaluations, develop individualized plans of care, and implement therapeutic interventions to improve functional mobility, reduce fall risk, and support patient independence. You will serve a diverse, fast-moving patient population with strong interdisciplinary collaboration.

Responsibilities

• Complete PT evaluations and determine individualized goals and care plans.

• Deliver therapeutic exercises, ambulation training, balance activities, and functional interventions.

• Update treatment plans as patient conditions evolve.

• Document evaluations, progress notes, and discharge summaries clearly and promptly.

• Communicate regularly with nursing, social services, and therapy colleagues.

• Educate patients and caregivers on safe mobility and home programming.

• Utilize clinically proven PT techniques to enhance outcomes.

• Participate in case conferences and care plan updates.

• Maintain compliance with facility and state standards.

Experience / Qualifications

• Active California PT license.

• Prior SNF experience encouraged, not required.

• Strong clinical reasoning and evaluation skills.

• Professional communication and documentation abilities.
