POSITION: Physical Therapist - Sign on Bonus $5000.00 DUTIES: Completes screening process of all patients. Evaluates and treats all patients who could benefit from intervention as referred by an approved referral source. Appropriately document progress toward identified physical therapy objectives. Communicate patient status and needs to the patient, patients family, relevant staff members and other professionals included in the care plan of the patient. Actively integrates therapists role into patient care team. Provides education, guidance and support of other health care professionals and the general community about physical therapy. Make appropriate referrals to other members of the rehabilitative team. Develop and implement new programs/techniques to enhance physical therapy services and facility utilization. Adheres to established facility policies and procedures related to environmental safety, orderliness, risk management, infection control and emergency situations. Maintain awareness of issues related to the profession of physical therapy and the healthcare environment. Perform other functions as required to fulfill the expectations of the Physical Therapist role. REQUIREMENTS: - Masters degree in Physical Therapy accredited by the American Physical Therapy Association (APTA). - At least two years of previous work experience as a Physical Therapist is required. OTHER REQUIREMENTS: - An acceptable combination of education, training and experience that provides the above knowledge, abilities and skills may be substituted. - Necessary Special Requirements: Must obtain annual flu and Covid-19 vaccination and any job-specific immunizations. - Required to submit to tuberculosis screening process. - Licenses or Certificates: If driving, a valid drivers license and ability to be insured as a facility driver. Valid Colorado Physical Therapist license. SKILLS: - Ability to maintain an efficient work schedule with a high level of productivity. - Ability to utilize problem-solving skills to work with patients, families, staff members and other team members to aid in providing high quality care. - Ability to operate general office equipment including computer software applications (e-mail, surveillance equipment/cameras). - Excellent verbal and written communication skills. - Possesses advanced active listening skills, including seeking clarification when in doubt and requesting feedback when needed. - Ability to establish and maintain effective relationships with the public, other agencies, employees and administration.
