Connected Health Care is seeking an experienced Physical Therapist (PT) for a local travel assignment in San Jose, California. Job Description:- Evaluate, plan, and implement individualized treatment plans for patients
- Use evidence-based practices to restore function, relieve pain, and improve mobility
- Educate patients and families on home exercise programs and prevention strategies
- Collaborate with physicians, nursing staff, and other therapists to ensure quality care
- Document patient progress and update treatment plans accordingly
- Maintain compliance with all local, state, and federal regulations
Education & Certification Requirements:- Degree in Physical Therapy from an accredited program Required
- Current California PT License Required
- BLS Certification Required
- Minimum of 1 year of clinical experience preferred
