As a physical therapist, you will provide skilled physical therapy services in patients’ homes, including evaluations, individualized treatment planning, and ongoing collaboration with physicians and an interdisciplinary care team. You will work with a diverse patient population including post-surgical recovery, neurological conditions, orthopedic injuries, and chronic disease management. In this role, you will support patients across Souderton, Lansdale, Telford, Perkasie and Quakertown areas

Physical Therapist (PT) Responsibilities:

  • Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care. Evaluate the home environment.

  • Assess for muscle strength, mobility, gait, ROM—potential for rehab.

  • Direct physical therapy treatment.

  • Instruct patients and families/caregivers in the use and care of therapeutic appliances.

  • Determine priority needs for physical therapy

  • Report to physicians on patients’ reaction to treatment or changes in condition.

  • May train patients in the use of prosthetic devices.

  • Identify patient and family/caregiver needs for other home health services and referrals as necessary.

  • Prepare and submit clinical and progress summaries based on the attainment of goals.

  • Participate in discharge planning for patients.

  • Prepare and submit a clinical progress summary based on the attainment of goals directed by Organization policy.

  • Provide physical therapy consultation to home families/caregivers when indicated.

  • Provide service education programs for nursing organization personnel as needed.

  • Participate in the peer consultation process.

  • Ensure that arrangements for equipment and other necessary items and services are available.

  • Instruct, supervise and evaluate physical therapy aide care provided every fourteen days.  

Required Qualifications:

  • Current Physical Therapist license in the state of Pennsylvania

  • Graduate of an accredited Physical Therapy program

  • Strong clinical assessment, communication, and documentation skills

  • Valid driver’s license and reliable transportation

  • Current CPR/BLS certification

  • Minimum 1 year of clinical experience; home health experience?
