Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home health experience with adult and geriatric patients
• Experience with OASIS
• Experience with EPIC
• Active Illinois PT license
• BLS from AHA
• Valid driver's license
• Auto insurance
• Reliable transportation
• Ability to assist with ADLs and adaptive equipment
• Ability to collaborate with other specialties

Assignment Details

• Assignment length: 13 weeks
• Schedule: Monday through Friday
• Shift hours: 8:00 AM - 5:00 PM
• Orientation includes approximately 2 days onsite in Oak Brook, followed by remote work within the designated coverage area
• Position is remote within the designated coverage area after orientation
• No weekend rotation
• No call
• No holiday expectations
• Expected patient volume is 5 to 6 visits per day
• Floating within the specified coverage area may be required
• Traveler must provide transportation to and from patient visits
