Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active North Carolina Physical Therapist license required to start the assignment.
• Current BLS certification from AHA or ARC required prior to start.
• Experience in home health preferred. Candidates with prior Oasis documentation experience preferred.
• Experience performing admissions, evaluations, treatment visits, 30-day reassessments, resumption of care, and recertifications preferred.
• Must be able to travel within a multi-county coverage area (coverage includes visits across five counties).
• Minimum education requirement: Bachelor's degree in Physical Therapy or higher.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days 0800-1600 (8:00 AM–4:00 PM).
• Typical Schedule: Monday through Friday, 8:00 AM–4:30 PM per hiring manager notes.
• Weekly Hours: 40 hours guaranteed.
• Mileage reimbursement provided between patient visits, from first patient to last patient.
• Primary responsibilities: Provide skilled physical therapy in the home health setting, including admissions, evaluations, ongoing treatments, 30-day reassessments, resumption of care, and recertification when indicated.
• Required professional certification: MPT or DPT and BLS.
• Dress code: Navy scrubs.
