Job Overview
Royal Oaks is seeking a compassionate and motivated Physical Therapist to join our rehabilitation team on either a full-time or per diem basis. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide high-quality therapy services that promote mobility, independence, safety, and overall quality of life.
This position offers the opportunity to work in a collaborative healthcare environment serving older adults in a skilled nursing, assisted living, and/or outpatient setting.
Responsibilities
- Evaluate residents/patients and develop individualized physical therapy treatment plans
- Provide therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence
- Establish measurable goals and regularly assess patient progress
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary team members
- Educate residents, families, and caregivers regarding treatment plans and home exercise programs
- Complete accurate and timely documentation in accordance with state, federal, and facility guidelines
- Maintain compliance with all Medicare, Medicaid, and regulatory standards
- Participate in care conferences and discharge planning as needed
- Ensure a safe and supportive therapy environment
Qualifications
- Graduate of an accredited Physical Therapy program
- Current Iowa Physical Therapist license in good standing
- CPR certification preferred
- Experience in skilled nursing, long-term care, outpatient, or rehabilitation settings preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
