Clinical Care – Home Health Physical Therapist (PT)
Location: Willits / Ukiah, CA
Schedule: Full-Time | Monday–Friday | 8:00 AM – 5:00 PM

Seeking an experienced Home Health Physical Therapist to provide individualized in-home therapy services and collaborate with an interdisciplinary care team.

JOB SUMMARY

The Physical Therapist plans and delivers individualized treatment programs to restore and improve patients’ physical functioning. Works collaboratively within an interdisciplinary team to ensure quality patient outcomes.

REQUIREMENTS:

Education & Experience:

• Bachelor’s Degree or equivalent – Required

• Master’s Degree – Preferred

• Home Health experience – Required

Licenses & Certifications:

• Current Physical Therapist (PT) license in the state of California – Required

• Valid Driver’s License and must be at least 21 years of age – Required

• Basic Life Support (BLS) or CPR certification from AHA– Required

Responsibilities:

• Conduct comprehensive patient evaluations and assessments

• Develop individualized treatment and discharge plans

• Provide skilled therapy interventions using appropriate protocols and techniques

• Maintain timely, accurate, and compliant clinical documentation

• Communicate care plans effectively with patients, families, and care teams

• Supervise Physical Therapist Assistants as assigned

• Perform other duties as assigned
