The Physical Therapist provides comprehensive evaluation and treatment services to improve patients’ mobility, strength, function, and overall quality of life. This role focuses on delivering patient-centered, holistic rehabilitation care, with an emphasis on treating adult and geriatric populations with complex medical and functional needs. The PT collaborates with an interdisciplinary care team across clinical and community-based settings.
Key Responsibilities:
Perform physical therapy evaluations and develop individualized treatment plans
Implement therapeutic interventions to improve mobility, strength, balance, and functional independence
Monitor and document patient progress and adjust care plans as needed
Collaborate with physicians, nurses, and rehabilitation staff to coordinate patient care
Educate patients and caregivers on exercises, mobility techniques, and fall prevention strategies
Provide care across outpatient, acute, inpatient, or skilled nursing settings as assigned
Participate in interdisciplinary care planning for complex patient populations
Ensure compliance with clinical standards, safety protocols, and regulatory requirements
Required Qualifications:
Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred)
Valid California Physical Therapist License or proof of license eligibility
Minimum of two (2) years of clinical experience preferred, particularly with frail or elderly populations
CPR and First Aid certification within 180 days of hire (preferred upon hire)
Preferred Experience:
Outpatient rehabilitation experience
Acute care experience
Inpatient rehabilitation experience
Skilled nursing facility (SNF) experience
Strong experience working with geriatric patients
Required Certification:
California Physical Therapist License
Ideal Candidate Profile:
Strong geriatric rehabilitation experience
Experience in outpatient or acute care settings
Open to interdisciplinary, holistic care environments
Pediatrics-only experience will not be considered
Strong commitment to functional, patient-centered outcomes
