Description

A Physical Therapist (PT) evaluates, develops, and implements individualized treatment plans to improve patients' mobility, strength, function, and overall quality of life in a home health setting. The Physical Therapist collaborates with physicians and interdisciplinary teams to provide evidence-based care, ensure regulatory compliance, and promote safe, independent living while maintaining accurate clinical documentation and high standards of patient care.

Job Responsibilities

  • Provide physical therapy services in accordance with state licensure and practice standards.
  • Evaluate patients and develop individualized treatment plans based on physician orders.
  • Deliver skilled therapeutic interventions to improve mobility, function, and independence.
  • Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as required.
  • Collaborate with physicians and interdisciplinary teams to coordinate patient care.
  • Educate patients and caregivers on home exercise programs and safe mobility techniques.
  • Complete OASIS assessments, recertifications, transfers, and discharge documentation.
  • Maintain accurate clinical documentation using appropriate ICD-10 coding.
  • Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.
  • Ensure compliance with HIPAA, infection control, and patient safety standards.
  • Document patient progress, care coordination, and treatment outcomes accurately.
  • Attend interdisciplinary meetings and continuing education programs.
  • Deliver compassionate, patient-centered care in home and community settings.
  • Utilize EMR and organizational technology systems for documentation and communication.
  • Demonstrate professionalism, flexibility, and excellent customer service.
  • Perform additional duties as assigned while adhering to organizational policies.

Qualifications

• Graduate of an accredited physical therapist curriculum approved by the Commission on 

Accreditation in Physical Therapy Education, the American Physical Therapy 

Association, or the Committee on Allied Health Education and Accreditation

• Currently licensed as a Physical Therapist in the state of practice

• Two years of clinical experience preferred; home health experience a plus but not required

• Basic computer proficiency; willingness to learn EMR systems and OASIS documentation

• Current CPR certification
