Minimum Requirements:
- Associate's degree in physical therapy from an accredited educational program.
- Current Wisconsin PTA license.
- BLS Certification from AHA or ARC may be required.
- Experience working in skilled nursing or with older adult populations preferred.
- Experience with Net Health documentation is a plus.
- Strong communication and teamwork abilities. Flexibility and professionalism required.

Your Responsibilities
- Provide physical therapy treatments under PT supervision in a skilled nursing facility setting.
- Implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
- Support residents in achieving personal goals and rehabilitation milestones.
- Accurately document care and maintain compliance standards.
- Collaborate with therapy and nursing teams to ensure patient-centered outcomes.
- Educate residents and families on safe techniques and strategies to maximize independence.
- Participate in team meetings and contribute to care planning.
