Minimum Requirements:

• Associate's degree in physical therapy from an accredited educational program.
• Current Wisconsin PTA license.
• BLS Certification from AHA or ARC may be required.
• Experience working in skilled nursing or with older adult populations preferred.
• Experience with Net Health documentation is a plus.
• Strong communication and teamwork abilities. Flexibility and professionalism required.

Your Responsibilities

• Provide physical therapy treatments under PT supervision in a skilled nursing facility setting.
• Implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Support residents in achieving personal goals and rehabilitation milestones.
• Accurately document care and maintain compliance standards.
• Collaborate with therapy and nursing teams to ensure patient-centered outcomes.
• Educate residents and families on safe techniques and strategies to maximize independence.
• Participate in team meetings and contribute to care planning.
