Responsibilities May Include:

  • Participate as needed with in office or telephonic case conferences with the Interdisciplinary team.
  • Coordinate Assessment findings and DC planning with all members of the HH treatment team.
  • Attend required training on HH clinical programs
  • Attend all in-services and training related to HH regulatory changes and PT practice related to our current HH polices and procedures.
  • To instruct other health care team personnel in certain phases of physical therapy with which they may work with the patient.
  • Counsel patients to help them understand their disorder.
  • Instruct the patient and family in care and use of physical therapy devices and all aspects included in the patient's plan of care.
  • Work with patients of all ages.
  • Provide physical therapy services as prescribed by a physician which can be safely provided in the home.
  • Evaluate patients by applying diagnostic muscle, nerve, joint and functional abilities tests (PT only).
  • PTA will progress a patient using appropriate treatment plan outlined and established by a PT.
  • Work with teachers, physicians, psychologists, social workers, and other helping professionals to address patient mobility and other physical therapy related disorders.
  • PT will supervise and provide mentorship to the PTA as outlined in the Brooks Home Care Advantage Policy and Procedures.

Education and Qualifications

  • Current licensed PT/PTA in the state of Florida with a degree from an accredited institution
  • Must have reliable transportation to and from patient homes
  • Current CPR certificate.
