Home Health Physical Therapy Assistant

We are seeking a compassionate and motivated Home Health Physical Therapy Assistant to join a dedicated healthcare team focused on delivering exceptional patient care in the comfort of patients’ homes. As part of the care team, you will assist in implementing patient treatment plans while helping patients improve mobility, strength, and overall quality of life throughout Lebanon, PA and surrounding communities.

Responsibilities

• Assist in the delivery and management of individualized patient care plans under the supervision of a licensed Physical Therapist

• Provide compassionate in-home therapy support to patients in Lebanon, PA and nearby areas

• Collaborate with interdisciplinary healthcare teams to support patient recovery goals

• Accurately complete charting and documentation within required timeframes

• Effectively manage daily schedules and patient visits independently

• Monitor patient progress and communicate updates to supervising therapists and care teams

• Help create a positive and supportive patient care experience

Qualifications

• Current Physical Therapy Assistant (PTA) license in the state of Pennsylvania required

• Previous home health or healthcare experience preferred

• Strong communication and organizational skills

• Ability to work independently and manage time efficiently

• Comfortable using electronic charting and mobile technology

• Valid driver’s license and reliable transportation required

Schedule

• Monday through Friday

• 8:00 AM – 5:00 PM

• No weekends or on-call requirements

Travel Requirements

• Local travel to patient homes required throughout the day

• Approximately 20% of the workday spent driving between patient visits
