About the Job
As a Physical Therapist Assistant in Homecare, you’ll deliver skilled, compassionate rehabilitative care directly in the patient’s home. Working under the supervision of a Physical Therapist, you’ll implement individualized treatment plans, reinforce therapeutic exercises, and help patients regain mobility, safety, and independence. This role blends handsâon clinical skill, patient education, and strong communication to support goalâdirected, costâeffective care in the home setting.
What You’ll Do
- Implement physical therapy treatment plans in collaboration with the supervising Physical Therapist.
- Reinforce home exercise programs by educating patients, families, and caregivers after discharge from PT services.
- Monitor and communicate changes in patient status, progress, or goals to the supervising PT.
- Coordinate care by effectively communicating with the interdisciplinary team and contacting the team manager as needed.
- Educate patients and caregivers on proper body mechanics and safe mobility techniques.
- Document timely, accurate clinical notes in the electronic medical record.
- Support daily operations by being flexible with scheduling and traveling within the agency’s territory in all seasons.
- Participate in staff meetings, inâservices, and required trainings.
- Provide excellent customer service and contribute to positive patient outcomes.
- Perform other duties as assigned.
Who You Are
- A Physical Therapist Assistant with an Associate’s degree in Physical Therapy.
- Registered and in good standing with the New Hampshire Board of Nursing.
- Equipped with at least two years of hospital or clinic experience (homecare experience preferred).
- Certified in CPR and able to meet criminal background and motor vehicle record requirements.
- Skilled in implementing treatment plans, educating patients, and communicating effectively with the care team.
- Comfortable working independently in the home setting while collaborating closely with a supervising PT.
- Competent with electronic medical records and timely documentation.
- Compassionate, adaptable, and committed to delivering highâquality, patientâcentered rehabilitative care.
