Position Summary

We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist, you will provide skilled therapy services that help residents improve mobility, strength, function, and independence.

Key Responsibilities

  • Provide physical therapy treatments as directed by the supervising Physical Therapist
  • Implement individualized treatment plans based on physician orders and therapy goals
  • Monitor resident progress and communicate updates to the Physical Therapist
  • Document treatment sessions, progress notes, and resident outcomes accurately and timely
  • Assist with discharge planning and caregiver education
  • Collaborate with nursing staff and interdisciplinary team members to support positive resident outcomes
  • Participate in care conferences and team meetings as needed
  • Maintain a safe, clean, and organized therapy environment
  • Ensure compliance with all state, federal, and facility regulations
  • Help residents achieve their highest level of function and independence

Qualifications

Education & Experience

  • Graduate of an accredited Physical Therapist Assistant program
  • Experience in skilled nursing, rehabilitation, or long-term care preferred
  • Experience with PointClickCare (PCC) and Casamba preferred
  • Strong communication, documentation, and organizational skills

Licenses & Certifications

  • Current South Carolina Physical Therapist Assistant (PTA) license in good standing
  • Must maintain all required continuing education requirements
  • Must remain in good standing with the State Board
