Skilled Nursing Facility (SNF) in
San Jose, California, to recruit a dedicated and compassionate
Physical Therapist Assistant (PTA) for a

Job Responsibilities- Implement physical therapy treatment plans developed by the Physical Therapist
- Assist patients in improving mobility, strength, balance, endurance, and functional independence
- Provide therapeutic exercises, gait training, transfer training, and mobility interventions
- Monitor patient progress and accurately document treatment outcomes
- Educate patients and families on exercise programs, mobility techniques, and injury prevention
- Collaborate with physicians, physical therapists, nursing staff, and other healthcare professionals
- Maintain compliance with facility policies, state regulations, and professional practice standards
- Support patients in achieving maximum functional mobility and independence
- Maintain accurate and timely documentation within the electronic medical record (EMR)
Education & Certification Requirements- Associate Degree from an accredited Physical Therapist Assistant (PTA) program
- Current California Physical Therapist Assistant (PTA) license required
- Current BLS (Basic Life Support) certification required
- Previous Skilled Nursing Facility (SNF) experience preferred
- Strong communication, documentation, and patient care skills
- Ability to work collaboratively within an interdisciplinary rehabilitation team
