Responsibilities

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans

• Provide hands-on therapeutic care and functional mobility training

• Collaborate with interdisciplinary rehab teams (OT, SLP, Nursing)

• Document all treatments and progress per facility and regulatory standards

• Educate patients, families, and caregivers on recovery techniques and exercises

Requirements

• Active California PT license

• Current BLS Certification (AHA-approved)

• Prior SNF or post-acute rehab experience preferred (new grads encouraged to apply)

• Strong communication and documentation skills

Compensation & Benefits

• $45–$50/hour (depending on experience)
