    Job Description

    Purpose: The Street Medicine Advanced Practice Provider (APP) delivers compassionate, patient-centered medical care to individuals experiencing homelessness and other vulnerable populations in community-based settings. The APP works collaboratively with an interdisciplinary team to provide primary care, chronic disease management, urgent care services, behavioral health support, care coordination, and health education directly in the field.

    This role requires flexibility, strong clinical judgment, cultural humility, and a passion for serving underserved communities through outreach-based healthcare delivery.

    Responsibilities and Duties:

    • Provide comprehensive primary and urgent care services in nontraditional settings including shelters, encampments, community centers, and outreach locations

    • Perform patient assessments, diagnose conditions, develop treatment plans, and prescribe medications within scope of practice

    • Manage chronic illnesses such as hypertension, diabetes, COPD, and mental health conditions

    • Assess and support patients experiencing substance use disorders through harm reduction strategies, medication-assisted treatment (MAT) when appropriate, patient education, and referrals to recovery resources and community support services

    • Conduct health screenings, preventive care visits, and follow-up appointments

    • Coordinate care with physicians, behavioral health providers, social workers, case managers, and community partners

    • Assist patients with access to medications, insurance enrollment, transportation, housing resources, and specialty referrals

    • Maintain accurate and timely documentation in the electronic medical record (EMR) system

    • Participate in outreach events, community engagement activities, and interdisciplinary case conferences

    • Support harm reduction initiatives and trauma-informed care practices

    • Ensure compliance with organizational policies, HIPAA regulations, and clinical standards

    • Assist in quality improvement efforts and program development initiatives

    Qualifications

    Required

    • Current Alabama licensure as a Nurse Practitioner (NP) or Physician Assistant (PA)

    • DEA registration and prescriptive authority eligibility

    • Current BLS certification

    • Minimum of 1-2 years of clinical experience in primary care, urgent care, family medicine, emergency medicine, or community health

    • Strong communication and interpersonal skills

    • Ability to work independently in community-based environments

    • Valid driver's license and reliable transportation

    Preferred

    • Experience working with underserved or unhoused populations

    • Experience in community outreach, street medicine, addiction medicine, or behavioral health integration

    • Experience providing care or support for individuals with substance use disorders preferred

    • Bilingual abilities are a plus

    • Knowledge of trauma-informed care and harm reduction principles

    Physical Requirements

    • Ability to travel to various outreach locations throughout the service area

    • Ability to walk, stand, bend, and lift supplies/equipment as needed during outreach activities

    • Ability to work outdoors in varying weather conditions

    Work Environment

    This position involves both clinic-based and community-based work. The APP will regularly provide care in mobile or field settings and may encounter unpredictable environments and situations requiring adaptability and sound clinical judgment.
