    Job Requirements

    The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for the Advanced Heart Failure Service provides comprehensive, evidence-based care for patients with advanced heart failure, mechanical circulatory support devices, cardiogenic shock, and heart transplant needs. Working collaboratively with heart failure cardiologists, cardiac surgeons, pharmacists, nurses, and multidisciplinary team members, the APP manages patients across the continuum of care in the inpatient setting, focusing on optimization of guideline-directed medical therapy, patient education, care coordination, and improved clinical outcomes.

    Key Responsibilities

    • Perform comprehensive patient assessments, including detailed history, physical examination, and evaluation of cardiovascular status.
    • Diagnose, manage, and monitor patients with advanced heart failure, reduced and preserved ejection fraction heart failure, pulmonary hypertension, cardiomyopathies, and cardiogenic shock.
    • Initiate, titrate, and optimize guideline-directed medical therapy (GDMT) in accordance with current clinical guidelines.
    • Manage patients with mechanical circulatory support devices, including left ventricular assist devices (LVADs), and provide support for heart transplant evaluation and follow-up.
    • Order, interpret, and act upon diagnostic studies, including laboratory testing, ECGs, echocardiograms, hemodynamic monitoring, and cardiac imaging.
    • Participate in inpatient rounding, consultation services, and transitions of care planning.
    • Coordinate multidisciplinary care involving cardiology, cardiac surgery, palliative care, social work, rehabilitation, and community providers.
    • Provide education and counseling to patients and families regarding disease progression, treatment options, medication adherence, lifestyle modifications, transplant, and LVAD therapies.
    • Facilitate timely outpatient follow-up, symptom management, and prevention of avoidable hospital readmissions.
    • Document clinical encounters and treatment plans in the electronic medical record in accordance with organizational and regulatory requirements.
    • Participate in quality improvement initiatives, research activities, and program development efforts for advanced heart failure services.

    Work Experience

    • Master's or Doctoral degree from an accredited Nurse Practitioner or Physician Assistant program.
    • Current state licensure as a Nurse Practitioner or Physician Assistant.
    • National certification appropriate to specialty.
    • DEA registration and prescriptive authority as required.
    • BLS certification required; ACLS certification required or obtained within designated timeframe.
    • Experience in cardiology, heart failure, critical care, or cardiovascular medicine preferred.
