About the Role
You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:
• Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day • Collaborating with interdisciplinary teams to optimize patient outcomes • Ensuring smooth transitions of care with hospice palliative care and home care agencies • Maintaining accurate and timely documentation in our EMR system

Qualifications
Graduate degree as a nurse practitioner or physician assistant
Active and unrestricted NP or PA license in North Carolina
Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal abilities
Experience in post-acute care preferred
Compensation is fee for service (FFS). Expected compensation is estimated range of $93,000 to $149,000 annually with no cap on productivity income potential. This full-time role is benefit eligible.
