Physician Assistant / Nurse Practitioner – Palliative Care Opportunities (New Jersey)

A growing, mission-driven palliative care organization is seeking Physician Assistants and Nurse Practitioners to join its expanding clinical teams across New Jersey. These roles focus on delivering compassionate, high-quality care to patients with serious illness while working within a strong interdisciplinary and supportive care model.

We are currently hiring for two distinct opportunities:

1) Inpatient Palliative Care – Jersey City, NJ

This hospital-based role is located at Jersey City Medical Center and focuses on inpatient palliative care consultations across the ED, ICU, and medical/surgical units.

This position requires prior palliative care experience and is ideal for clinicians who thrive in high-acuity hospital settings and complex decision-making environments. You will be supported by a strong clinical infrastructure, structured mentorship, and a collaborative interdisciplinary team.

Key focus areas include:

  • Inpatient palliative care consultations across hospital units
  • Goals-of-care discussions and advance care planning
  • Complex symptom management in acute care settings
  • Coordination with hospitalists, specialists, and care teams
  • Integration into established hospital workflows and protocols

2) Outpatient / SNF & Community Palliative Care – Ocean County, NJ

This role covers skilled nursing facilities and community-based settings across Ocean County, New Jersey.

It offers a structured ramp into palliative care, making it well-suited for clinicians with strong experience in serious illness care from backgrounds such as oncology, geriatrics, hospital medicine, critical care, SNF/LTC, or family/internal medicine. Prior palliative care experience is helpful but not required.

Key focus areas include:

  • Palliative care assessments and follow-up visits in SNFs and community settings
  • Management of patients with advanced illness and complex chronic conditions
  • Goals-of-care and advance care planning conversations
  • Collaboration with SNF teams, community providers, and hospice agencies
  • Care transitions from hospital to SNF, home, or hospice settings
  • Local travel within Ocean County with support and mileage reimbursement

What Both Roles Offer

  • Competitive base salary: $130,000–$160,000/year
  • Annual performance bonus up to 12.5%
  • Monday–Friday schedule with flexibility
  • 28 PTO days in year one, increasing to 33 days at year two

  • License and DEA support and renewals
  • CME allowance up to $1,000/year

Ideal Candidate Profile

  • Active New Jersey PA or NP license
  • National board certification and DEA registration (or eligibility)
  • Strong communication and interpersonal skills
  • Ability to work collaboratively within an interdisciplinary team
  • Experience in palliative care preferred for inpatient role; optional for outpatient role
  • Background in serious illness care strongly valued
