Our client seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing primary care team. The ideal candidate will provide high-quality, patient-centered care in a collaborative environment focused on prevention, chronic disease management, and improving patient outcomes.

Essential Duties and Responsibilities

  • Conduct comprehensive patient assessments, examinations, and evaluations.
  • Diagnose and treat acute and chronic medical conditions.
  • Order, review, and interpret laboratory tests, imaging studies, and other diagnostic procedures.
  • Develop and implement individualized treatment plans.
  • Prescribe medications and therapies in accordance with applicable regulations and organizational policies.
  • Provide preventive care services, annual wellness visits, and patient education.
  • Manage chronic conditions including hypertension, diabetes, obesity, COPD, asthma, and other common primary care diagnoses.
  • Review and address laboratory results, imaging reports, patient messages, and clinical tasks in a timely manner.
  • Maintain  accurate, complete, and timely medical documentation.
  • Coordinate care with specialists, hospitals, and ancillary healthcare providers.
  • Participate in quality improvement initiatives and organizational programs designed to improve patient outcomes.
  • Maintain compliance with all federal, state, payer, and organizational requirements.

Requirements

  

Required

  • Current Michigan Nurse Practitioner or Physician Assistant license, or eligibility to obtain prior to employment.
  • Current DEA registration or eligibility to obtain.
  • Current BLS certification.
  • Strong communication, interpersonal, and clinical decision-making skills.
  • Commitment to providing exceptional patient-centered care.

Preferred

  • Experience in Primary Care, Family Medicine, Internal Medicine, Urgent Care, or Geriatric Medicine.
  • Experience utilizing electronic medical records.
  • Knowledge of value-based care and quality metrics.

Compensation and Benefits

  • Base salary: $105,000 – $120,000 annually
