    Compensation:$120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

    Position Summary

    Most Physician Assistant opportunities look similar on paper.

    As a Wound Care & Geriatric Rounding Provider, you'll step into a highly specialized role that combines advanced wound management, geriatric medicine, chronic disease management, and community-based care. Rather than spending your day moving room-to-room in a traditional clinic, you'll become a trusted clinical resource throughout senior living communities while helping patients navigate complex medical needs.

    Responsibilities

    • Provide on-site rounding, primary care, and advanced wound care services.

    • Manage pressure injuries, diabetic ulcers, surgical wounds, skin integrity concerns, and complex chronic conditions.

    • Perform assessments, diagnostics, treatment planning, and follow-up care.

    • Drive chronic disease management and preventative care initiatives.

    • Collaborate with physicians, nurses, therapists, caregivers, and facility staff.

    • Help reduce avoidable hospitalizations through proactive clinical intervention and continuity of care.

    • Complete documentation, medication management, and lab coordination.

    Qualifications

    • Active Minnesota Physician Assistant (PA-C) license in good standing.

    • Experience in wound care, geriatrics, family medicine, internal medicine, primary care, or post-acute settings preferred.

    • Strong clinical judgment and confidence managing medically complex patients.

    • Valid driver's license and reliable transportation.

    • Passion for delivering outcomes-driven, patient-centered senior care.
