Position Summary:
This is a standout opportunity for a Physician Assistant (PA-C) who wants to do more than follow a routine. As a Wound Care and Rounding Provider, you will take ownership of complex clinical cases, lead treatment decisions, and become a trusted expert across your communities. You will see your patients consistently, track progress over time, and directly influence healing outcomes. This role is built for clinicians who want to elevate their career, expand their expertise, and operate with true clinical impact.

Responsibilities:
• Provide on-site rounding and advanced wound care services across senior living communities.
• Manage high-acuity wounds including pressure injuries, surgical wounds, and diabetic ulcers.
• Drive chronic care management and preventative strategies to improve outcomes.
• Partner with physicians, nurses, and facility leadership to coordinate care.
• Act as a clinical leader and resource within each community you support.
• Communicate with families and caregivers to ensure clarity and alignment.
• Complete documentation, medication management, and lab coordination.

Qualifications:
• Active Minnesota Physician Assistant (PA-C) license in good standing.
• Experience in wound care, geriatrics, post-acute care, or primary care preferred.
• Strong ability to manage complex patients and make independent clinical decisions.
• Valid driver’s license and reliable transportation.
• Passion for delivering high-quality, outcomes-driven senior care.

Military Occupational Classification (MOC) Codes:
• Army: 65D – Physician Assistant.
• Navy: Physician Assistant (230X).
• Air Force: 42G3 – Physician Assistant.
• Marines: Physician Assistant (230X).
• Coast Guard: Health Services Officer (HSO) / Physician Assistant.
