Role Overview
Reporting to the Medical Director, the Primary Care Physician will provide coordinated and integrated care for patients with a wide range of medical needs. Responsibilities include evaluating and treating patients, managing acute and chronic conditions, ordering and interpreting diagnostic tests, prescribing treatment, administering vaccinations, providing follow-up care, and educating patients on disease prevention and health maintenance.
The physician will also support quality improvement initiatives, participate in care coordination, maintain accurate medical records, and collaborate closely with clinical and administrative teams to support the organization’s mission, vision, and values.
Key Responsibilities
- Provide comprehensive primary care to patients of all ages
- Diagnose and manage acute, chronic, and complex medical conditions
- Develop treatment plans based on evidence-based medical protocols
- Perform focused and comprehensive health assessments based on patient needs
- Order and interpret diagnostic testing
- Prescribe medications in accordance with Connecticut statutes and professional guidelines
- Provide preventive care, vaccinations, screenings, and patient education
- Monitor patient progress and adjust treatment plans as needed
- Refer patients to specialists or higher levels of care when appropriate
- Maintain accurate, timely, and complete medical records
- Participate in quality improvement, quality assurance, and utilization review activities
- Support Accountable Care Organization activities as required
- Participate in policy, procedure, and workflow improvement
- Work as part of a multidisciplinary clinical team
- Participate in on-call rotation as required
- Support care delivery across other sites as needed
Ideal Candidate
The ideal candidate will bring:
- MD or DO degree from an accredited medical school
- Completion of accredited residency training in an appropriate specialty
- Board certification or board eligibility preferred
- Current or eligible Connecticut medical license
- Valid federal and state controlled substance licenses
- Primary care or ambulatory care experience preferred
- Experience serving low-income, multicultural, or underserved populations valued
- Strong interest in team-based, community-focused care
- Ability to care for patients across the lifespan, including infants, adolescents, adults, and geriatric patients
- Strong communication, documentation, and interpersonal skills
- Working knowledge of ICD-10, CPT, HCPCS coding, and managed care preferred
- Valid driver’s license and ability to travel within the service area as needed
