What You'll Do
Provide comprehensive primary care, including preventive, acute, and chronic disease management
Build meaningful, long-term relationships with patients and families
Collaborate with Whole Health Coaches and care team members to develop personalized care plans
Use evidence-based medicine and population health strategies to improve patient outcomes
Empower patients through education, wellness coaching, and shared decision-making
Qualifications
MD or DO from an accredited medical school
Board Certified or Board Eligible in Family Medicine, Internal Medicine, or a related primary care specialty
Active Georgia medical license or ability to obtain prior to employment
DEA registration or ability to obtain
Passion for patient-centered, relationship-based care
