Provide comprehensive outpatient primary care patients 65 and older.
Managing chronic conditions and developing personalized treatment plans
Performing preventative care, wellness visits, and routine screenings
Collaborating with nurse practitioners, specialists, and care teams to optimize patient outcomes
Maintaining accurate and timely electronic health records documentation
Participating in quality improvement initiatives to enhance patient care and clinic operations
Staying up to date on evidence-based medical practices and advancements in primary care
Engaging in community outreach and health education efforts when applicable

Requirements
Board Certified or Eligible in Family Medicine, Internal Medicine, or Med-Peds
At least 5 years of post-residency clinical experience
Strong clinical and diagnostic skills
Commitment to patient-centered care and building lasting relationships
Good communication and team skills
Interest in working in a community-focused practice
Willingness to work some early mornings, evenings, and rotating weekends
Outpatient primary care experience (preferred)
