Lead Nephrologist – Value-Based Care | Hybrid (Atlanta, GA)

An innovative, national healthcare organization is seeking a Board-Certified Nephrologist to serve as a Lead Physician in a forward-thinking, value-based care model for patients with chronic kidney and end-stage renal disease.

This is an exciting opportunity to join a multidisciplinary team transforming the delivery of kidney care through data-driven, patient-centered, and home-based clinical services.

Position Highlights
  • Full-time, Monday–Friday schedule

  • No nights, weekends, or call

  • Hybrid model: 60% remote (telehealth), 40% in-person home visits

  • Primarily clinical responsibilities

  • Variable daily volume (average 3–7 patients/day via telehealth)

Key Responsibilities
  • Deliver comprehensive nephrology care for patients with chronic and end-stage kidney disease in home and telehealth settings.

  • Partner with interdisciplinary teams to develop and implement value-based initiatives focused on improving outcomes and reducing costs.

  • Utilize evidence-based practices and technology platforms to enhance care quality, data tracking, and performance outcomes.

  • Engage patients in education and shared decision-making to promote long-term kidney health.

  • Participate in multidisciplinary case reviews and continuous improvement initiatives.

Qualifications
  • MD or DO from an accredited institution

  • Board Certification in Internal Medicine and Nephrology

  • Active, unrestricted medical license and DEA registration

  • Strong clinical experience in nephrology across inpatient, outpatient, and dialysis settings

  • Excellent communication skills and collaborative leadership style

  • Experience or interest in value-based care and population health strongly preferred
