Primary Care Physician
This role is ideal for a physician who wants to provide comprehensive care to older adults in a team-based environment. You will work closely with an interdisciplinary care team to support patient needs across clinical, home, and nursing facility settings.
What Youll Do
Perform initial, semi-annual, and annual assessments for participants
Lead interdisciplinary team meetings and guide individualized care plans
Provide primary medical care across clinic and long-term care environments
Collaborate with Nurse Practitioners, Physician Assistants, and other clinical team members
Support hospital coordination, care transitions, and follow-up planning
Document care in the electronic health record and follow established clinical guidelines
What Youll Need
M.D. or D.O. degree with active medical licensure
Board Certification or Board Eligibility in Internal Medicine or Family Medicine
Experience working with older adults or medically complex populations
Ability to work effectively in a coordinated, team-based care model
DEA registration and current Basic Life Support certification
Strong communication, clinical judgment, and patient-focused decision-making
