    Position Summary

    The physician is responsible for providing compassionate and quality medical care within the standards of the hospital, to their patients. Shall exert their best efforts and devote their time and attention to the practice of medicine, within the realm of their specialty.

    Essential Duties and Responsibilities:

     

    1. Admits, treats and discharges patients to the hospital in accordance with established policies/procedures. Prepares and signs hospital medical records and related reports in a timely manner.  Completes and submits all encounter forms on a timely basis.  Insures appropriate level of coding and complies with government and third party regulations regarding documentation, including teaching physician rules.
    2. Communicates effectively with patients. Communicates health status and treatment plan to patient and/or family in a way that is clearly understood. Responds to patient’s needs including responding to patient phone calls in a timely manner.
    3. Assures that medication is properly prescribed and documents this information in the patient chart; Physician shall also explain usage and side effects of medications to patient. Reviews and initials all incoming labs, X-rays and consultative reports making necessary adjustments to patient treatment.
    4. Physician shall cooperate with other physicians employed by the Hospital in regard to providing services to patients and on-call schedule. Physician will also attend regularly scheduled office hours, mutually agreed upon by HPH and Physician.
    5. Participates in regular medical staff activities and staff responsibilities including attendance at required meetings, in-service education, staff development, and quality management activities including but not limited to: development of indicators, data collection, results interpretation and action plans for improvement.
    6. Demonstrates concern for quality performance and exercises initiative to suggest change/ improvements with the scope of performance responsibility. Reports incidents pursuant to HPH’s policy and completes appropriate documentation, including HPH’s and Clinic’s QA/Risk Indicator forms.
    7. Attends all hospital mandatory in-services and education days. Participates in developing and maintaining quality management activities.
    8. Performs duties according to approved policies and procedures relating to Joint Commission, OSHA, Universal Body Fluid presentations and other regulatory body standards. Complies with HPH Clinic guidelines and policies/procedures.
    9. Submits inpatient and outpatient charges with appropriate documentation on a timely basis. Seeks methods for cost containment and acts upon them.

     

    Other Responsibilities:

     

    1. Adheres to downtime procedures.
    2. Adheres to established departmental policies, procedures, and objectives.
    3. Performs both essential and non-essential job functions in a safe manner as identified by Humboldt Park Health.
    4. Demonstrates an understanding of and models the mission and core values of Humboldt Park Health.
    5. Enhances professional growth and development by accessing educational programs, job related literature, in-service meetings, and workshops/seminars.
    6. Performs other related duties as assigned.
