    Responsibilities

    Clinical Care

    • Provide comprehensive outpatient primary care services to adult and/or family medicine patients
    • Conduct patient evaluations, preventive care visits, chronic disease management, and acute care assessments
    • Develop and implement treatment plans in collaboration with patients and interdisciplinary team members
    • Coordinate care with specialists and other healthcare providers across the health system
    • Promote evidence-based care and population health initiatives

    Patient Education & Support

    • Educate patients and families regarding diagnoses, treatment plans, preventive health measures, and wellness strategies
    • Encourage patient engagement and shared decision-making
    • Support patients in navigating healthcare resources and services

    Documentation & Information Management

    • Maintain accurate, timely, and compliant medical records
    • Utilize electronic medical records efficiently and effectively
    • Ensure patient confidentiality and adherence to regulatory requirements

    Quality & Professional Standards

    • Demonstrate professionalism, integrity, accountability, and compassion in all interactions
    • Participate in quality improvement and patient safety initiatives
    • Adhere to organizational policies, medical staff bylaws, and regulatory standards

    Professional Development

    • Maintain clinical competency through continuing medical education
    • Participate in professional development opportunities and quality initiatives
    • Collaborate with colleagues throughout Yale New Haven Health to advance patient care and clinical excellence

    Qualifications

    Required Qualifications

    • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
    • Completion of an ACGME-accredited Internal Medicine or Family Medicine residency program
    • Board Certified or Board Eligible in Internal Medicine or Family Medicine
    • Commitment to patient-centered, high-quality primary care

    Licensure Requirements

    • Valid and unrestricted Connecticut medical license or eligibility to obtain one
    • Board certification in Internal Medicine or Family Medicine (or board eligibility if newly trained)
    • Current DEA registration or eligibility to obtain
    • Controlled Substance Registration
    • Current BLS certification
