Tristate Health Pay, LLC is seeking Physician in Secaucus, NJ with support in North Bergen, NJ to provide comprehensive, evidence-based primary and preventive care to adult patients within a Tristate outpatient practice. In this role, you will evaluate and manage acute and chronic conditions, coordinate complex care, and partner with the interdisciplinary team to advance patient safety, clinical quality, continuity of care, and operational excellence.

- 8:30AM to 5PM EST
- Potential Saturday coverage as necessary

The right candidate will have:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution, or an equivalent medical degree that meets applicable licensure and credentialing requirements.
- Board certification in Internal Medicine, or board eligibility with certification obtained within the timeframe established by Tristate.
- Preferred five or more years of experience providing adult primary care in an outpatient Physician practice setting.
- Preferred five or more years of experience managing medically complex patients and multiple chronic conditions.
- Experience with value-based care, quality improvement, population health, clinical leadership, or supervision/education of clinical team members.
- Current DEA registration and any state controlled-substance registration required for assigned duties, or eligibility to obtain.
- Current BLS certification and any additional life-support certification required by the practice or assigned responsibilities.
Our Physician will:
- Conduct comprehensive and focused assessments, obtain histories, perform physical examinations, and diagnose and manage acute and chronic medical conditions.
- Develop, implement, and monitor individualized treatment plans consistent with current evidence, professional standards, and patient goals.
- Provide preventive care, annual wellness visits, chronic disease management, episodic care, and post-hospital or transitional care for adult patients.
- Order and interpret laboratory, imaging, and other diagnostic studies and communicate findings and recommendations clearly and promptly.
- Prescribe medications and other therapeutic interventions in accordance with licensure, privileges, applicable law, and organizational policy.
- Perform office-based procedures for which the physician is trained, competent, and privileged.
- Recognize and manage urgent or complex presentations and arrange emergency care, consultation, referral, or a higher level of care when appropriate.
- Educate patients and families and support shared decision-making, prevention, adherence, and effective self-management.
- Coordinate care with specialists, hospitals, advanced practice providers, pharmacies, and community resources to support safe transitions and continuity.
- Collaborate with and provide appropriate clinical consultation to physicians, advanced practice providers, nursing staff, medical assistants, and other care team members.
- Contribute to evidence-based clinical protocols, peer review, case review, and quality or safety initiatives.
- Promote a respectful, accountable, team-based culture and reinforce appropriate clinical workflows, escalation practices, and professional standards.
- Manage the assigned patient schedule, results, messages, and other clinical work efficiently while maintaining quality and patient-centered care.
- Support organizational goals related to preventive care, chronic disease outcomes, patient safety, patient experience, access, population health, and value-based care.
- Comply with credentialing, privileging, peer review, privacy, infection-prevention, controlled-substance, billing, and other legal or organizational requirements.
- Maintain professional competence through continuing medical education, certification, and required training.
