Help ensure patients receive the coordinated, compassionate care they deserve. As a Physician Services Clinical Coordinator, you'll serve as the central point of connection between providers, patients, caregivers, and healthcare partners, helping navigate complex healthcare needs and ensuring care plans are carried out smoothly and efficiently. Through thoughtful coordination, communication, and follow-through, you'll help create a seamless care experience for elderly and medically complex patients.
This role is ideal for someone who thrives on organization, problem-solving, and building meaningful relationships while finding purpose in supporting patients and families. You'll coordinate services, manage provider orders, facilitate communication between care teams, support patient follow-up, and connect patients with the resources needed to support their health and well-being.
