About the Position
This is a time limited position. The MSUD Medical Director provides clinical and medical leadership for the MSDHs office-based treatment of opioid use disorder (OUD) delivered both via telehealth and in-person through county health departments statewide. The MSUD Medical Director serves as the collaborating physician for agency nurse practitioners (APRNs), performs required chart reviews and quality assurance activities, delivers direct patient care for clinically complex OUD cases, and ensures program compliance with the Mississippi Board of Nursing and the Mississippi State Board of Medical Licensure rules, DEA/HHS telemedicine policies, and 42 CFR Part 2/HIPAA privacy standards. Maintaining current written collaborative practice agreements with MSDH APRNs practicing within the MSUD Program, ensuring the agreements define the scope, formulary, emergency procedures, and responsibilities as per state rules and regulations. Conduct monthly chart reviews of a random sample representing 10% or 20 charts (whichever is less) for each collaborating APRN; ensure reviews represent the variety of patient types; document findings and corrective actions. Participate in and document quarterly face-to-face (or approved virtual) QA/QI meetings with collaborating APRNs -Ensure that an ongoing QA/QI program for APRN practice is maintained on-site and available for Board inspection, supporting audit readiness. Serve as medical policy director for MSUD clinical practice; perform peer review to ensure compliance with standards of care and regulatory requirements. Ensure adherence to state rules regarding collaboration logistics and maintain current rosters and coverage plans. Provide telehealth-based evaluation and treatment for higher-acuity or clinically complex patients with OUD and common co-occurring conditions (e.g., polysubstance use, pregnancy/postpartum, unstable housing) -Initiate and manage buprenorphine-based treatment and other FDA-approved MOUD, including induction, stabilization, maintenance, and medically supervised taper when indicated; coordinate extended-release formulations and naltrexone as appropriate. Ensure comprehensive discharge planning and stepped-care transitions that account for the patients home environment, supports, and self-management skills. Practice in accordance with current federal requirements. Maintain compliance with DEA/HHS telemedicine rules and 42 CFR Part 2/HIPAA; ensure secure, office-to-office telehealth workflows at MSDH county health departments. Chair the MSUD Clinical Governance/QA meeting; review safety events, near misses, UDS/diversion monitoring, and adherence to standardized care pathways (e.g., same-day initiation, dose adjustments, overdose risk mitigation). Approve and periodically update clinical protocols (induction strategies, pregnancy/perinatal OUD care, co-prescribing guidance for benzodiazepines and stimulants, pain management for patients on MOUD, hepatitis C testing/treatment referral, and naloxone distribution). Provide vision and leadership to further the mission of the program, improving clinical care, and optimizing existing EHR workflows. Ensure appropriate use of the Mississippi Prescription Monitoring Program (PMP) per state policy and integrate PMP checks into EHR workflows. Oversee co-signature processes when required by rule, payer, or program policy. Provide case consultation to APRNs, nurse navigators, pharmacists, and social workers, maintaining availability for consultations during clinic hours and scheduled complex-case conferences. Lead training on MOUD best practices, perinatal OUD, stimulant co-use, overdose prevention, and regulatory updates (DEA/telemedicine, MSBN/MSBML). Support credentialing/privileging and onboarding of new clinicians; verify licensure, DEA, and required trainings (MATE Act, CE for controlled substances). Define key clinical metrics (e.g., time-to-initiation, 30/90-day retention, dose optimization, UDS concordance, naloxone distribution) and review dashboards with program leadership. Collaborate with data teams to validate clinical data used for grants, legislative reporting, and public dashboards. Ensure timely completion of encounter documentation, prior authorizations, and medical necessity statements to support program billing and sustainability. Coordinate with hospitals and acute care institutions, community mental health centers, ED based programs, FQHCs, residential treatment programs, and other relevant organizations for seamless warm handoffs and effective stepped-care transitions. Represent MSDH in state and national forums on OUD treatment and telehealth regulations.

Preferred Qualifications
Requires licensure and typically 1-4 years of post-residency experience. Agency preferred qualifications: MD/DO with an unrestricted Mississippi medical license -DEA registration in good standing; MATE Act 8-hour SUD training completed/attested (one-time requirement). 3+ years of post-residency clinical experience, including MOUD via office-based telehealth. Eligibility to enroll with Medicaid/Medicare; no OIG exclusions. Demonstrated experience collaborating with APRNs and conducting formal QA/QI. Experience leading statewide or multi-site telehealth programs. Expertise in perinatal OUD, stimulant co-use, and complex comorbidity management. Epic EHR experience and proficiency with integrated PDMP workflows. Demonstrated ability to provide organizational and managerial oversight -Board certification in Family Medicine, Internal Medicine, or Emergency Medicine, with substantial experience in OUD.
