    Part-time Physician – Post-Acute Care | Flexible Schedule | Wharton, TX

    Trumen Physicians + Associates is seeking a dedicated and experienced Physician to join our growing post-acute care team in Wharton, TX.     About the Role:

    As a Post-Acute Care Physician, you’ll manage a panel of patients in skilled nursing and rehabilitation facilities. You’ll play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.

     

    Core Responsibilities:

    • Provide in-person medical care to patients in assigned post-acute facilities

    • Perform assessments, manage treatment plans, and update documentation in the EMR

    • Lead or collaborate with facility teams to ensure quality outcomes and smooth transitions of care

    • Participate in Trumen’s value-based care and transitional care programs

    • Develop strong working relationships with facility staff and leadership

    • Serve as a liaison between Trumen and facility teams

    • Be available in assigned facilities anytime during regular business hours on scheduled days

     

    Qualifications:

    • MD or DO with a current, unrestricted Texas medical license

    • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine

    • 1+ year of experience in post-acute, geriatric, or SNF care preferred

    • Comfortable practicing independently in skilled nursing facilities

    • Strong clinical judgment, communication skills, and a patient-centered mindset

    • Reliable transportation within the Southwest Houston area
