About the Role:
Skilled Nursing Pharmacy in Hayward, CA is looking for a dedicated and knowledgeable Staff Pharmacist to join our team. Responsibilities:

• Dispense and verify prescription medications accurately for skilled nursing facility residents
• Perform drug utilization reviews (DUR) and ensure medication therapy appropriateness
• Collaborate with nursing staff and physicians to optimize patient medication regimens
• Monitor for drug interactions, contraindications, and adverse drug reactions
• Ensure compliance with state and federal pharmacy regulations and long-term care guidelines
• Counsel and support facility staff on medication administration and safety protocols
• Maintain accurate patient medication records and documentation

Requirements:

• Active California Pharmacist license (RPh) in good standing
• Doctor of Pharmacy (PharmD) degree from an accredited institution
• Experience in long-term care, skilled nursing, or institutional pharmacy preferred
• Strong knowledge of geriatric pharmacotherapy and medication management
• Familiarity with California Board of Pharmacy regulations and OBRA guidelines
• Excellent attention to detail and strong organizational skills
• Effective communication and interpersonal skills for multidisciplinary collaboration
