About the Role:
Join Skilled Nursing Pharmacy as a Staff Pharmacist in Hayward, CA, where you will play a vital role in providing exceptional pharmaceutical care to our patients. We are looking for a dedicated professional who is passionate about improving patient outcomes and delivering high-quality service in a collaborative environment.

Schedule:
- Start Times Vary from 1:00pm to 6:00pm.


Responsibilities:
  • Dispense medications accurately and in a timely manner, ensuring patient safety.
  • Conduct medication therapy management and patient consultations.
  • Collaborate with healthcare providers to optimize patient medication regimens.
  • Monitor patient medication therapies and identify potential drug interactions.
  • Maintain accurate and up-to-date patient records and medication profiles.
  • Provide education and support to patients and their families regarding medications.
  • Participate in quality assurance initiatives and adhere to regulatory standards.
  • Assist in training and mentoring pharmacy technicians and interns.
Requirements:
  • PharmD degree from an accredited pharmacy school.
  • Active California pharmacist license in good standing.
  • Minimum of 1 year of experience in a pharmacy setting, preferably in long-term care.
  • Strong communication skills and a patient-centered approach.
  • Ability to work collaboratively in a fast-paced environment.
  • Knowledge of pharmacy software and electronic health records.
  • Commitment to continuous learning and professional development.
  • Detail-oriented with strong analytical and problem-solving skills.
