THE JOB AT A GLANCE
This position will report to the Area Manager and will be primarily clinic-based. There may be some travel to other health clinics. Candidates should reside in one of these areas: San Francisco (Civic Center, Irving Street), East Bay (Alameda, Albany, Berkeley), or South Bay (Santa Clara, Campbell, San Mateo).
The Care Coordinator is responsible for managing and coordinating patient care across primary care and research studies to ensure optimal patient outcomes. This role involves working closely with patients, healthcare providers, and other external payer stakeholders to facilitate access to necessary services, and provide continuous support throughout the healthcare journey.
WHAT YOU’LL DO
Referrals and Authorizations Coordination
Assist in coordinating referrals and authorizations for specialist care, diagnostics, imaging, etc, ensuring that these services are covered and that the patient’s care continuum is seamless and well-managed.
Care coordination
Engage with IPA or HMO payers, members and patients to outreach and schedule appointments for patients due for critical preventative care and facilitate access to necessary services.
IPA patients utilizing UC service – coordinating care for establishing care in PC
Patient outreach and scheduling
Responsible for clinical research duties and tasks, outreach and engage with patients to ensure patients are scheduled for clinical research
Provider coordination and patient engagement
Manage provider task messages or boxes, ensuring timely responses, appropriate task delegation, and accurate documentation.
ABOUT YOU
Strong organizational and time-management skills.
Excellent written and verbal communication skills.
Proficient in Google Suite (Sheets and Document).
Strong problem-solving abilities and attention to detail.
Compassionate and patient-centered approach to care.
1+ years experience working with HMO or IPA payers or responsible for care coordination functions.
