Your Valued Contributions:

The Population Health Coordinator provides clinical and operational leadership for care management, community health, and transition-of-care programs. This role ensures standardized workflows, high-quality patient outcomes, regulatory compliance, and optimal performance in value-based care initiatives.

  1. Clinical Oversight & Care Management
  • Ensure development, implementation, and documentation of patient-centered care plans
  • Provide clinical guidance for high-risk and complex patients
  • Oversee risk stratification processes and interventions
  • Conduct routine case reviews and interdisciplinary case conferences
  1. Staff Supervision & Development
  • Direct supervision of day-to-day work and coaching
  • Competency assessments annually
  • Support onboarding and training of new staff
  • Monitor caseload distribution and productivity
  1. Transitions of Care Management
  • Ensure completion of Care Management outreach and enrollment
  • Monitor and address readmission trends
  1. Workflow & Program Standardization
  • Develop and implement standard operating procedures (SOPs)
  • Ensure consistent documentation across EMR
  • Standardize CHW engagement and referral workflows
  • Improve cross-site consistency
  1. Data, Reporting & Performance Improvement
  • Monitor and report on:

  • UDS clinical quality measures

  • HEDIS measures

  • Hospital admissions and readmissions

  • Emergency department utilization

  • Develop and maintain dashboards

  • Lead quality improvement initiatives

  1. Compliance & Regulatory Oversight
  • Ensure documentation meets requirements for:

  • HRSA

  • Value-based contracts

  • Care management billing (CCM, TCM)

  • Support internal audits and external reviews

  • Ensure adherence to the scope of practice and program integrity

  1. Interdisciplinary Collaboration
  • Collaborate with:
  • Primary care providers
  • Behavioral health teams
  • Operations
  • Health Information
  • External partners (hospitals, ACOs, community organizations)

Required Training & Experience:

  • Use of Electronic Health Records.
  • Word & Excel.
  • Care management, population health, or case management experience.
  • Possesses a current State of Michigan driver's license and valid automobile insurance.
  • Active RN license (preferred; equivalent experience considered if non-RN).
  • Possesses a current Basic Cardiac Life Support (BCLS) CPR certification or obtain within six (6) months of start of employment.
