Job Summary
The Population Health Nurse will work in partnership with patients, families, and interdisciplinary care teams to deliver high-quality, cost-effective care aligned with value-based care (VBC) models, including ACO, Medicare Advantage, and commercial risk arrangements. This role is responsible for proactive population health management, including risk stratification, chronic disease management, care gap closure, and care transitions. The Population health Nurse provides evidence-based clinical coaching to support optimal performance on ACO and payer-specific quality metrics.
Requirements: Must have a valid Minnesota Registered Nurses license. At least two (2) years' experience in caring for chronic disease preferred. Prefer experience in provider practice or clinic health setting. Previous Care Coordination, Case Management, Home Health or Behavioral Health experience preferred. Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred. Ability to identify and implement appropriate patient communication strategies. Previous experience with health IT systems and data reports preferred.
Wage
Depending on your work experience, you could earn an annual salary of $78,064 up to $113,194.
