We provide long-term solutions to the problems of homelessness, addiction, incarceration, and poverty by helping the people we serve overcome obstacles and develop healthier, more self-sufficient lives.

    Case Manager - CBP

    SUMMARY:

    The case manager provides support services to individuals and families experiencing homelessness to secure and maintain stable housing and household income while enrolled in the VOA Permanent Supportive Housing Program.

    ESSENTIAL JOB FUNCTIONS:

    • Provide all aspects of Housing Case Management for a case load in accordance with VOA policies and HUD and community standards.

    • Establish a strong working relationship with Rochester Housing Authority (RHA) and collaborate with RHA staff to provide HUD required services to mutual participants.

    • Receive and respond to referrals within 48 hours.

    • Complete and update SCN through Find Help with every client, follow up on referrals as needed. Enhanced Care Management through VOA referrals requires regular updates and notes.

    • Update case management notes in SCN and reimbursement trackers in Find Help.

    • Schedule intake, confirm eligibility and provide necessary documentation to RHA for processing.

    • Assist with collection of recertification documentation and provide to RHA.

    • Conduct regular in‑person sessions with participants, including comprehensive home visits at least once per month. Provide additional check‑ins as needed to ensure ongoing support and engagement.

    • Must be able to conduct home visits and meet with clients independently unless supervisor(s) assigns otherwise.

    • Develop an individualized service plan for new participants.

    • Complete and update assessment and individualized service plan on biannual basis.

    • Complete and update acuity indexes on a quarterly basis.

    • Responsible for ensuring participants locate and maintain stable housing.

    • Responsible for ensuring participants obtain, maintain or increase income.

    • Create and maintain a good working relationship with landlords.

    • Provide on-going and relevant pro-social supports and life skills training.

    • Use evidence‑based practices to support participants in identifying and addressing barriers to stability.

    • Provide transportation to participants as needed.

    • Establish good working relationships with related agencies to maintain a comprehensive referral network for participants.

    • Maintain accurate records for all participants in accordance with VOA policies and relevant regulatory and funding sources, including but not limited to use of HMIS.

    • Prepare and submit reports, outcome data, and other documentation as requested by supervisor(s).

    • Other duties as assigned.

    EDUCATION & QUALIFICATION REQUIREMENTS:

    • Associates Degree in Human Services or related field with relevant work or life experience preferred.

    • Bachelor's degree and at least one year experience preferred.

    • Clean NYS Driver's License

    • Strong organizational skills

    • Strong working knowledge of Microsoft Office software

    • Flexibility required to respond to emergency client needs.

    • Availability to occasional work evenings and weekends as required
