MAIN FUNCTION:The PrEP Navigator, under the direction of the Program Director, medical provider, and PrEP Specialist, provides support and advocacy to patients prescribed pre-exposure prophylaxis for HIV prevention. The PrEP Navigator will provide a bridge between providers and patients that facilitates the medical and psychosocial care of the individual.The role of the PrEP Navigator is to assist in the coordination of care. The Navigator will serve as a reliable source of clinic information, appropriate resources, and HIV prevention information. The PrEP Navigator will maintain a presence in the community through the use of social media and/or public outreach. The Navigator will correspond with local businesses to participate in events and provide various outreach materials and will assist potential patients who respond to outreach, in scheduling and accessing clinic HIV prevention services.Administrative ResponsibilitiesAttend educational opportunities including HIV 101, Motivational Interviewing, NYS PrEP Trainings, as well as STI and Hepatitis C risk reduction education. Participate in support group meetings, case conferences, and Quality Improvement and/or Quality Management teams. Input encounters, as appropriate, into reporting systems.Act as public outreach liaison, maintaining a community presence. Follow up with patients lost to care via phone, text, or mail, as directed by the PrEP Specialist.Patient Engagement ResponsibilitiesAssist patients in scheduling appointments by addressing barriers and identifying strengths.Assist patients in completing forms when necessary.Re-enforce care management instructions and treatment plans.Co-facilitate treatment education activities EDUCATION AND EXPERIENCE:High School Diploma or GED. CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS: HeartsaverREQUIRED QUALIFICATIONS: Knowledge of Pre-exposure Prophylaxis therapy for HIV prevention. Familiarity with community-based services in the Southern Tier of NY Ability to work as part of a team, with other Navigators at our Agency and with health care providers in clinical settings. Honesty and genuine compassion for individuals living with HIV/AIDS. Ability and willingness to accept direction from supervisor. Good oral and written English communication skills. Good telephone skills Ability to be comfortable with the diversity of our community. Ability to maintain required work schedule, be on time, keep work area neat, be accountable for how time is managed/used. Understanding of Article 27F HIV Confidentiality Laws. Ability to advocate for clients by bringing concerns about services to providers' attention. Ability to help clients identify risk reduction strategies (safer sex, drug treatment, needle exchange, etc.) Strong knowledge of HIV disease, treatments, and substance abuse issues. PREFERRED QUALIFICATIONS: Basic computer proficiency Prior experience with record keeping. Training certificate in; HIV 101, Peer Education/Advocacy, PrEP therapy, etc. are preferred but not required. PHYSICAL DEMANDS: Light Physical Effort. Routine office responsibilities
