Statement of Purpose: This position provides testing, linkage and navigation linkage services to individuals for HIV prevention treatment, including specialized HIV prevention education, sexual health risk assessment, addressing barriers to accessing treatment, and monitoring retention. \n \nPrimary Tasks/Responsibilities:\n\n Conduct HIV and HCV testing in office and in a community based mobile health setting.\n Conduct sexual health risk assessment with patients and assess level of risk and appropriateness for PrEP.\n Provide linkage to additional supportive services for HIV negative patients on PrEP/nPEP treatment\n Identify and mitigate patient barriers to access and retention in HIV prevention treatment.\n Assess client's health insurance status, assist clients with insurance enrollment, complete prior authorizations, enrollments, and pharmaceutical company medication assistance as needed to ensure access to healthcare and medication treatment.\n Coordinate referrals to essential and supportive services identified during sessions with patients such as: linkage to care, mental health or substance abuse counseling, primary care, STI treatment, case management, support or social groups, etc.\n Complete accurate and immediate documentation of navigation services in electronic medical records system, along with ongoing timely documentation of all patient contacts and communication.\n Work succinctly with medical providers, members of patient's treatment team, and other agency personal to ensure patients' seamless access to onsite PrEP/nPEP treatment.\n Maintain required records, files, and forms in an organized manner and in accordance with HIPAA and other state, local, and federal guidelines\n Complete specialized training specific to HIV prevention programs' design, implementation, and behavior change theory application.\n Travel between New Port Richey and Port Richey locations.\n Other relevant related duties as assigned.\n \nEducation/Professional:\n\n Bachelor's Degree in public health, social services or related field OR equivalent experience working in the field.\n A minimum 1 year of experience in the field and knowledge of HIV/STD infection, transmission and prevention; evidence-based interventions, with an understanding of health education principles and behaviors\n State certified HIV Rapid pre/post-test counselor within 60 days of hire. Phlebotomy certification preferred.\n Bilingual (English/Spanish) strongly preferred but not required.\n \nKnowledge, Skills and Abilities Required:\n\n Excellent verbal and written communication skills, including strong interpersonal and organizational skills.\n Working knowledge of community resources and local referrals for HIV/HCV/STI and additional appropriate client-centered referrals preferred.\n Ability to recognize and adhere to professional boundaries and standards.\n Ability to work independently and as part of an integrated team.\n Ability to work effectively and efficiently in a fast paced and changing environment, and skilled at multi-tasking.\n Strong computer skills and knowledge including word processing, database operations, spreadsheets, and other software systems, including electronic medical records (experience with eClinical Works preferred)\n Knowledge of or experience working with the LGBTQ and racial/ethnic minority communities and familiarity with issues of particular relevance to these populations\n Demonstrated ability to collaborate effectively with colleagues and deliver respectful, high-quality care to all patients in a community health setting across wide range of backgrounds and perspectives.\n \nRequirements:\n\n Must possess and maintain valid Florida driver's license and proof of insurance\n Must have reliable and accessible auto vehicle.\n* Must pass necessary fingerprinting, Level II background checks(https://flclearinghouse.com) and employment eligibility verification through the U.
