Description

CHARACTERISTICS OF THE WORK

Under general supervision performs nursing work.

Responsible for providing nursing care and accurate health information to clients in a primary care center under the auspices of the Family Health Centers (FHC).

Assesses patient and family health care needs, counsels patients and families, and participates in developing and implementing care plans.

Instructs patients and families in health education and disease prevention.

Examples of Duties

Assesses patient complaints via telephone and in person and recommends appropriate action according to prescribed protocol.

Performs nursing assessments and develops health care plans for patients.

Counsels patients regarding health status, risks, recommended actions and community resources.

Provides individual and group health education instruction and information.

Handles all abnormal and normal lab test results according to protocol.

Provides appropriate patient care based on age appropriate growth and development principles.

Participates in quality improvement activities.

Writes reports and policies and procedures in areas of expertise as assigned by Nursing Supervisor and Director of Nursing.

Mentors and acts as resource for health services students and department personnel.

Administers medications, injections, immunizations, venipuncture and treatments as ordered by provider or delegates same per protocol.

Documents patient history and assesses chief complaints.

Reviews charts and makes entries.

Assists providers with exams and procedures.

Prepares patient and room for appropriate examination procedures.

Assists other nursing staff personnel when appropriate.

Ensures a clean, well stocked and safe work area.

Keeps medication stocked and safe work area.

Orders and distributes supplies.

Adheres to standard precautions.

Travels between satellite clinics.

Performs related work as required.

Minimum Requirements

MINIMUM REQUIREMENTS

Graduation from a two year nursing school approved by the National League of Nursing.

Possession of a current license to practice professional nursing in the State of Kentucky. -OR- Possession of a work permit to practice professional nursing in the State of Kentucky by endorsement.

SPECIAL REQUIREMENTS

Must obtain CPR certification by the end of the probationary period and maintain certification throughout employment with FHC.

Must be able to provide own transportation.

Must wear protective equipment as mandated by OSHA.

Must be vaccinated for Hepatitis B, have provided evidence of immunization, or have signed a declination form within 10 days of initial assignment.

Must work varied work schedule.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

Moves and/or lifts more than 100 lbs. when positioning patients or when assisting patients on and off the examination table.
