Job Title: Primary Care Nurse Practitioner – Float/Travel Coverage
Location: Orange Park, FL (906 Blanding Blvd, Orange Park, FL 32065) — must be able to float to Macclenny, Callahan, and the Jacksonville surrounding area
Job Type: Contract to hire

Pay & Hours

• Pay Rate: $74.87/hr (W2)
• Hours: Approx. 30–38 hrs/week (variable scheduling; weekend coverage required)

Schedule (Clinic Hours / Shifts)

• Weekdays: 8:30 AM – 7:30 PM
• Saturday: 9:00 AM – 5:30 PM
• Sunday: 9:00 AM – 4:30 PM
• Expected mix: 2–3 weekdays + 6 out of 12 weekend shifts (Sat/Sun)
• Possible holiday rotations

Job Summary

Nurse Practitioners work collaboratively with a clinical care team to deliver evidence-based care and improve outcomes for a panel of patients. This is an in-person, patient-facing role and may require floating to nearby locations for coverage.

Key Responsibilities

• Provide primary/family care including assessment, diagnosis, treatment, and chronic condition management
• Deliver patient education, counseling, screenings, and wellness services
• Perform clinic administrative duties such as:
• • Opening/closing the clinic
  • Inventory and clinic upkeep/cleanliness
  • Patient follow-up
  • Verifying insurance and collecting payment
• Coordinate care with other healthcare professionals as needed

• Requirements

  • Active Nurse Practitioner (NP) license in the state of practice (active/in good standing)
  • Master’s degree from a Family Nurse Practitioner program
  • Current national board certification
  • BLS (AHA or ARC) required (must be active; copy required upon offer acceptance)
  • Minimum 1 year recent NP experience in primary care, family medicine, or urgent care
  • Must be willing to float to nearby clinics (Macclenny, Callahan, Jacksonville area)
  • DEA registration: may be required during onboarding (if applicable)
  • High School Diploma/GED (verifiable)
  • This role is safety-sensitive

  Nice to Have

  • Experience with common EMR/EHR systems
  • Willingness to occasionally travel beyond the immediate float radius (as needed)
