    Primary Care Nurse Practitioner

    Join Our Growing Team Bringing High-Quality Healthcare to Older Adults

    Senior Therapeutic Solutions is seeking a compassionate, motivated, and autonomous Primary Care Nurse Practitioner to provide comprehensive medical care to older adults living in assisted living communities throughout Michigan, primarily serving Ingham, Genesee, Lapeer, and Oakland County.

    This is an excellent opportunity for a provider who enjoys building meaningful relationships with patients while practicing independently in a supportive, collaborative environment. We are rapidly expanding our medical services and are looking for someone who wants to grow with our organization.

    Position Summary

    The Primary Care Nurse Practitioner will provide comprehensive primary care services within assisted living facilities, focusing on preventive care, chronic disease management, acute illness evaluation, medication management, and coordination with facility staff, families, specialists, pharmacies, and home health/hospice providers.

    This position begins as a flexible part-time 1099 contractor role with the expectation of transitioning into a full-time W-2 position as the caseload grows.

    Responsibilities

    • Perform comprehensive medical evaluations and routine primary care visits

    • Diagnose and treat acute and chronic medical conditions

    • Develop individualized treatment plans

    • Prescribe medications and monitor treatment effectiveness

    • Manage chronic illnesses including hypertension, diabetes, COPD, CHF, dementia, depression, anxiety, and other geriatric conditions

    • Complete required medical documentation accurately and timely within the electronic medical record

    • Coordinate care with assisted living staff, therapists, specialists, pharmacies, hospitals, home health agencies, and hospice providers

    • Review laboratory and diagnostic results

    • Provide education to patients, families, and caregivers

    • Participate in interdisciplinary care planning

    • Maintain compliance with Medicare, Medicaid, and commercial insurance documentation standards

    • Travel between assigned assisted living facilities

    Qualifications

    Required

    • Current Michigan Nurse Practitioner license in good standing

    • National Board Certification as a Nurse Practitioner

    • DEA registration (or ability to obtain)

    • Ability to prescribe controlled substances in Michigan

    • Eligible to credential with Medicare, Medicaid, and commercial insurance plans (or already credentialed)

    • Excellent clinical assessment and diagnostic skills

    • Strong communication and organizational abilities

    • Comfortable working independently

    • Reliable transportation and willingness to travel between facilities

    • Current driver's license and automobile insurance

    Preferred

    • Experience in primary care, geriatrics, internal medicine, family medicine, skilled nursing, assisted living, or long-term care

    • Experience caring for medically complex older adults

    • Familiarity with electronic medical records

    • Experience with Medicare documentation and billing
